LIFE's Adult Day Health provides daily services to adults who have physical or cognitive impairments such as:

Adult Day Health offers many benefits for participants. Seniors who attend our programs:

LIFE's Adult Day Health plays an important role in the continuum of care for older adults who would benefit from supportive services throughout the day.
LIFE's Adult Day Health serves older adults who:
Have Alzheimer's disease or other dementia
Have physical impairments
Have chronic health conditions that require routine management
Are socially isolated and/or experience depression
Need assistance with activities of daily living
Have chronic mental health disorders
Would benefit from not being home alone all day
Making the Referral: Your proactive referral will allow LIFE Senior Services to follow up and contact the family directly, increasing the likelihood that the services you recommend will be put in place.
LIFE's Outreach Coordinator and Family Assistance Coordinators offer personalized service in the following ways:
Sets up meetings with you, your patient and/or your patient's family during hospitalization or while receiving in-home care to discuss the many ways Adult Day Health can enhance the recovery process and provide caregiver support.
Contacts and follows up with your patient and their family caregiver after discharge to encourage follow-through on your recommendations.
Answers questions or arranges for a tour and up to three Free Visit Days to get the enrollment process started.

At LIFE's Adult Day Health, our programs and activities for seniors and adults with disabilities are designed to promote socialization, lifelong learning and days filled with purpose and self-esteem. Days are geared for each individual's needs and preferences and are designed to increase and maintain physical skills and abilities as well as provide an environment of engagement and fun for all participants:
Focus on wellness and fitness
Therapeutic individual and group activities
Activities adapted for all levels of abilities, including Alzheimer's/dementia
Bingo, card games of all types
Daily exercise classes including the Arthritis Foundation exercise class, taught by certified staff
Wii bowling, golf and fishing
Active games of skill (and chance) including horseshoes, bowling, Blongo and Frisbee golf
Current events, reminiscing, discussion groups, reading
Brain fitness activities
Computer games, internet access and computer classes with adaptive hardware and software for seniors and other adults
Gardening, cooking, woodworking, tinkering
Art, crafts, music
Community outings
Special guests, including therapy dogs, musicians, historians, children and many more
Special clubs including a men's group, book club and the Red Hat Society
Themed days (Hersey's Kiss Day, Veteran's Day, Western Day)
Person-Centered Approach
LIFE's Adult Day Health practices a philosophy of person-centered care, recognizing each person's abilities and potential. We provide a rewarding, positive social environment, responding to each person's mind, body and spirit.
An individualized care plan, based on interests, needs, abilities and personal goals is developed to ensure that participants receive the appropriate care, attention and services they need.

At LIFE's Adult Day Health, we provide personal care and basic health services designed to keep seniors and other adults healthy and safe while they spend their days with us, engaged in programs and activities. Caregivers have peace of mind, knowing that they are in a safe secure environment. Health services may include the following:
Vital signs and weights checked monthly (or more frequently if requested by a physician)
Assistance with personal care including toileting, bathing and eating
Physical assistance with ambulation, grooming, self-feeding
Rehabilitative services including physical, occupational and speech therapies
Nutritious (and delicious) meals and snacks
Special diets including liberalized diabetic, renal, and pureed
Medication administration
Glucose monitoring
Injections
Breathing treatments
Minor wound care treatment
Routine care for chronic illnesses
Person-Centered Approach
We are committed to person-centered care, recognizing each person's preferences, abilities and potential in a rewarding, positive social environment. Along with program participants and family members, our interdisciplinary team develops an individualized plan of care. This care plan is reviewed every six months. We believe that each person makes a difference and that everyone, regardless of age, should be independent and continue to grow and dream.
